Website Request Form

This form is used to request a new website, request new functionality to be added to an exisiting website, or to request a redesign of an existing website.  Our production team will discuss your request at their next meeting and a member of the Communication team will be in touch with you to discuss next steps.

New Website Information
The URL for is the address which a site can be reached at, for example: https://cfaes.osu.edu. CFAES websites have two basic options for URL addresses: something.osu.edu or something.cfaes.ohio-state.edu. .osu.edu addresses require approval from the University, this typically does not take long, but there are limitations. Common abbreviations and names of employees (i.e. smilthlab.osu.edu) will not be accepted by the university. If the URL needs to be transferred from an older site we will work with the University Hostmaster and facilitate the transfer. We allow .org addresses as well but these will need to be separately registered. .com, .net and other commercial URLs are not permitted on our websites.

Your department/unit will be responsible for creating, loading and maintaining all content on the website. Communications will provide training on loading the content and using the website program.

The footer of our websites contains contact information for the office/program the site represents. Please provide the contact information that should appear in the footer of this website.
Please list the name.n or email addresses of all users that are expected to need access to edit this website, each will need to attend a training session to gain access.
